What is Dry Weight among Patients with Renal Failure?

Dry weight is the ideal weight that patients want to have after dialysis. At this time, without swelling, patients have normal blood pressure and can breathe smoothly when lying. It is very important for doctors to evaluate the effect of dialysis patients take.
Dry weight is an approximate number given by doctors though two or four week’s observing the patient. Before dialysis, they will predefine a change of weight. If the patient have the symptoms like low blood pressure, fatigue, cramps and dizziness in or after dialysis, it predicates the number predefined is too low for the patient and the doctor will correct it in next dialysis. Accordingly, the symptoms like swelling and left heart failure show that the number is too high for the patient and the doctor will lower it next time. Though this adjusting, the suitable dry weight can be available.
That is why patients must weigh their weight before and after dialysis. What’s more, the weight of their clothes and shoes has to be taken into consideration. Without dry weight, the dewatering amount in dialysis is blind and will arouse many complications, such as low blood pressure in overdue dewatering or heart failure in incomplete dewatering.
After dry weight is fixed, attention must be paid to the increase of weight between two times of dialysis. As for patients with Renal Failure, water intake is usually more than it output, because of their oliguria or anuria. If the patients absorb too much water from daily meals, they also can have the symptoms like swelling or heart failure. What’s worse, in older to remove too much water, dialysis will remove so much weight that patients may feel uncomfortable. The symptoms of low blood pressure will occur. So it is better to control the ideal change of weight between two times of dialysis under 5%.

Sexual Life and Dialysis


Can Patients who undergo dialysis have sexual life? This is a common question we often meet. Generally, patients with stage 4 Renal Failure will have hyposexuality, because the pituitary gland can not receive the signals from testes and ovaries to produce sex hormones. And patients with end stage Renal Failure often have impotence and infertility in men, premature menopause in women, and low or totally absent sex drive in both genders. In addition, most patients with end stage Renal Failure have anemia, which is another negative influence on libido.
Large amount of toxins and excessive water can be discharged when patients just begin to take dialysis, so that physical function may return to normal level, which will be accompanied with normal physiological requirement. However, this condition will change during long term dialysis. In general, 35% male patients who are taking dialysis will have further decline of sexual function, the proportion of female patients is 25%. Only 9% male patients and 6% female patients will improve their sexual function.

Coffee is a brewed beverage with a dark, slightly acidic flavor prepared from the roasted seeds of the coffee plant, colloquially called coffee beans. The beans are found in coffee cherries, which grow on trees cultivated in over 70 countries, primarily in equatorial Latin America, Southeast Asia, South Asia and Africa. Green (unroasted) coffee is one of the most traded agricultural commodities in the world.[1] Coffee can have a stimulating effect on humans due to itscaffeine content. It is one of the most-consumed beverages in the world.

I believe that a lot of people fall in love with coffee, and I am one of them. However, coffee is hard on kidney when you already have Renal Failure. Some patients with Renal Failure strictly follow it. And some people may oppose to it. This is an old argument. Why do I say coffee is bad for patients with Renal Failure? There are three reasons as follow.
Ⅰ Coffee contains much caffeine, which is common sense. Many patients with Renal Failure maintain that caffeine has not bad effect on kidney disease, but we should not forget that coffee can always let spirit rouse, and then difficultly in falling asleep. Low quality of sleep and body can not get well rest, which may arouse the relapse of kidney disease.
Ⅱ Coffee restrains the absorption of ferrum, which is reported by dietitian. Patients with Renal Failure may know there will be renal anemia when Chronic Kidney Disease (CKD) develops into stage 3. At this moment, coffee plays a role in stopping the absorption of ferrum, which is bad for treating renal anemia

A Recipe for a Patient with Kidney Failure

Anushka Maraj, a patient with Kidney Failure, asked me which food she should have. So I advised a recipe for her according her medical conditions as following:
Creatinine clearance was 17.26ml/mins.
24 hour urinary protein was 342 mg/24h
The machine value was 21.6
No blood in my urine
No swelling
No shortness of breathe
Her creatinine is greatly lower than the normal level 80 to 120 ml/mins, which means she are about to be at the stage of the End Stage Kidney Failure. At this stage, the food plays an important role in retarding Kidney Failure deteriorating.
Protein As her 24 hour urinary protein is 342 mg/24h, a lot of protein is losing from her body. It is important to keep the balance between protein replenishing and reducing kidney workload. So her protein intake is controlled in 30g/d, which can be available from animal protein food such as red meat, peeled poultry, fish, milk and eggs. However, they must depend on the principle that each food should be had in a small amount.

Dialysis can't Treat Chronic Kidney Failure

Some patients with Chronic Kidney Failure misunderstand that dialysis can treat kidney disease so they don't use any other treatment methods. I should tell you here that dialysis is only a replacement therapy and it can't treat kidney failure.
If the Serum Creatinine is higher than 445umol/L, patients with Chronic Kidney Failure should take dialysis.
